It is important that you enter the same information on this form that you provided when you registered for the Hunter Education Class. If you cannot remember the exact information use as little as possible here. For instance, if you're not sure if you registered as Charles Smith III or Charles Smith or Chuck Smith, simply enter "Smith" for the last name and your date of birth.
