Request an ADA Accommodation

Program Accommodation Requests ?

Please forward your request to the appropriate division or office responsible for the program or activity in which you are seeking accommodation. You may contact the Office of Human Resources for assistance in determining the appropriate division or office to contact.

Accommodation Request Review ?

Requests for accommodation will be reviewed by the appropriate division or office. We will make every effort to meet the accommodation request(s) where possible. Accommodations not reasonably satisfied at the division or office level or in which the requestor is dissatisfied with the response and chooses to appeal, shall be forwarded to the FWC EEO/AA/ADA Committee for further review. The committee will make recommendations to the agency?s Executive Director, whose determination and decision shall be final. The agency shall make every effort to respond at each step within 45 days of receipt of the request.
